Company
Sponsored Team Registration Form:
If yours is a company-sponsored
team, please use this online form.
Standard Team Registration
Form:
If
your team is comprised of individuals (with no corporate sponsorship),
please use this online form.
(A $250 deposit is required
to reserve your spot in the league.)
Waiver Form: Everyone
on your team must fill out a waiver form, so please download
this document & make
copies for everyone.
