Company
Team Roster: If yours is a company-sponsored team, please fill
out this "Company Team Registration Form" version.
(The
authorizing person at your company must sign off on it.)
Standard Team Roster: If your team is comprised of individuals (with no corporate sponsorship),
please fill out this standard version.
(A $250 deposit is required
to reserve your spot in the league.)
Waiver Form: Everyone on your
team must fill out a waiver form, so please download this document & make
